Board Agreement

Agreement for the Provision of Accommodation and Individual Support Services at The Ryder-Cheshire Foundation Manawatu.

The intention of this agreement is to set out the obligations and guidelines to help ensure a happy board arrangement.

This agreement will only be changed with the written consent of both parties to the agreement.

Each party should retain a copy of this agreement.

Before signing this agreement all parties should carefully read it and seek advice if they are unclear about what they are agreeing to.

Parties to Board Agreements are subject to the provisions of the Privacy Act 1993.

The cost of your accommodation and support during your residency is set by the Ministry of Health or Accident Compensation Corporation. This payment is direct debited to The Ryder-Cheshire Foundation Manawatu from the Central Payment Office and WINZ or Accident Compensation Corporation. Your personal living allowance or ACC entitlement is paid from WINZ / ACC directly into your personal bank account.

(1) Eligibility

  1. You have been assessed by NASQ agency / ACC Case Manager in your area and have approval for a residential support subsidy / ACC residential services.
  2. You have been assessed as requiring a level of support of M of H 2, 3 , 4 or 5 or ACC R.R.S. 1 or 2
  3. You are between 18 and 64 years of age.
  4. There is a room available, in a house that is appropriate to your needs.
  5. You or your Agent have read, agreed and signed the terms and conditions set out herein.

(2) Termination of the Board Agreement may occur:

  1. If the funding agent(s) stop(s) payment for you residency.
  2. If in our reasonable opinion, your behaviour is disruptive, illegal or is offensive to other people who live or work at The Ryder-Cheshire Foundation Manawatu or other people in the community.
  3. If you have been reassessed by NASQ / ACC as no longer being appropriate for residency at The Ryder-Cheshire Foundation Manawatu.
  4. If you have been in hospital for more than 42 days.

The Ryder-Cheshire Foundation Manawatu ensures that a Resident will not be shifted from this facility unless:

  1. Requested by the Resident or their advocate/family on their behalf. Two weeks notification of termination is required to terminate this agreement.
  2. Assessed prior to being shifted by NASQ / ACC and with the involvement of any specialist support services.
  3. Two weeks notification of intent to terminate this Agreement by the Foundation is given in writing to you.
  4. Or as agreed by the MoH /ACC.

In a situation where the Resident voluntarily exits the facility, The Ryder-Cheshire Foundation Manawatu will notify NASQ / ACC as soon as possible or at least within 48 hours of exit.

(3) Service Agreement

What you will be entitled to as a Resident at The Ryder-Cheshire Foundation Manawatu:

  1. To live in a clean, well maintained and safe environment with no barriers to physical access
  2. Within 2 months of entry to the Foundation the development of a Personal Care Plan and an Individual Support Plan (to be reviewed and developed for each person when there is a significant change of needs or at least 6 monthly or at the request of the Resident)
  3. Assistance with personal cares as outlined in your Personal Care Plan.
  4. Assistance to reach goals as outlined in your Individual Support Plan
  5. Access to an Occupational Therapist.
  6. Lighting, heating, hot water and other utilities.
  7. You will be provided with a private, furnished room (linen provided) with shared bathroom/toilet facilities. (You are encouraged to bring your own personal effects, such as T.V. Video, bedroom furniture if you have them.)
  8. Each house has their own communal kitchen, laundry, dining and lounge room.
  9. Each house has a T.V. / DVD in its lounge.
  10. Access to kitchen and laundry facilities
  11. Assistance and encouragement to take responsibility and/or be involved with all personal and household activities of daily living
  12. A communal telephone is provided in each house; however, Residents may have a telephone installed in their own room at their own expense.
  13. The provision of nutritious and varied food in the form of three meals a day, plus morning and afternoon tea and supper. Any special dietary needs are also catered for.
  14. You will have access to appropriate medical care of your choice. Clear and recorded procedures for administering medication/medical treatments will be provided.
  15. Staff on active duty 24 hours per day.
  16. 24-hour access to an alarm system to summon emergency assistance when required.
  17. A vehicle with provision for transporting people in their wheelchairs if unable to transfer is available for use with a fee per kilometre payable.
  18. Assistance with managing your personal budget will be provided if necessary.
  19. There are scooters available for Residents use. Our Occupational Therapist will assess your safety in handling the scooters prior to these being available for your use.
  20. A social/activity group meets in our recreation hall Tuesdays and Thursdays, which you are welcome to attend.
  21. Weekly cleaning of your room and house where needed.
  22. Access to Community Health professionals.
  23. The services for each person will be detailed in their Personal Care Plan and Individual Support Plan.
  24. A suitably trained and qualified service provider will provide all of the services to you.
  25. You have rights as specified in the Code of Health and Disability Services Consumers’ Rights.

(4) Residents’ Responsibility

What The Ryder-Cheshire Foundation Manawatu is entitled to.

Levels of independence in daily living skills and personal hygiene skills vary according to the particular residential home at the complex being considered. Each person is assisted to develop an Individual Support Plan and Personal Care Plan that work towards maximising their potential level of independence. Assistance will be given with the personal activities of daily living, including cooking and other household tasks. Involvement in all tasks is encouraged.

Residents are expected to try to maintain or attempt to achieve as appropriate:

  1. Independence, both physically and generally
  2. Home management, cleanliness and basic household skills
  3. Educational and vocational pursuits
  4. Recreational activities
  5. Personal growth
  6. An ability to cope with a wide range of social situations
  7. A development of responsibility for one’s own actions
  8. A respect for other’s privacy
  9. An ability to relate to a wide range of people in a non-discriminatory way
  10. Behaving in a socially acceptable manner
  11. Consideration and tolerance of others on a care and share basis
  12. Co-operation with the health professionals who are treating you and provide them with relevant and accurate information about your health and medical history.
  13. Respect for The Ryder-Cheshire Foundation Manawatu property and a commitment to pay for any damage caused by yourself.
  14. Adherence to current health and safety practices and to your Individual Support Plan and Personal Care Plan

Residents will use the home in a similar way to any other accommodation. Residents are expected to exercise care and consideration for other residents and staff so that a healthy, positive and constructive atmosphere is fostered.

(5) What you are to provide or pay for from your personal funding

  • Personal clothing and toiletries including hand soap and toothpaste
  • Prescribed medications and medical equipment
  • Medical, recreational, entertainment, legal or educational expenses
  • Personal hygiene and grooming costs
  • Personal grocery items
  • Toll calls
  • Life and/or personal belonging’s insurance (optional purchase)
  • Transport costs
  • Pay to the Foundation any monies that you owe on account for any costs and expenses of the type referred, which have been incurred by The Foundation or its staff on your behalf.
  • Costs incurred by you as a result of damage to The Ryder-Cheshire Foundation Manawatu property.

(6) Disclosure/Privacy

Information is required so we can best serve your best interests as an individual. Your completion of the forms provided is required before you can be accepted for residency at The Ryder-Cheshire Foundation Manawatu. You hereby confirm that all material and information concerning your disability, state of health, health needs and medication requirements have been disclosed to the Foundation.

The Foundation will comply with the Privacy Act 1993 with regard to all information collected from you. You have the right to access and correct your personal information. It is your responsibility to advise us if any of the information collected becomes out of date or inaccurate, and therefore needs to be updated.
